How to order and
complete your Quest tests

• Go to the Labs section of our website.
• Choose the test or panel that fits your needs.
• Add it to your cart - each test can only be added once.
• A $10.00 Quest Diagnostics Draw Fee and a $9.50 Quest Diagnostics Physician Fee will be added automatically.
• Review your cart. If the same biomarker appears in both a panel and an individual test, we’ll let you know.
• Continue to Checkout.
• Enter your payment information.
• Enter the patient details (first name, last name, and email) for the person who will take the test.
• Complete your purchase.

• Your order will be processed by our partner, Fullscript.
• You’ll receive an email from Fullscript with a link to create or access your patient account.
• Once your lab requisition form (your official order) is ready, open My health → Labs.
• Find your test and select View details → View requisition to download the form.
• You’ll need to bring this form to your appointment, either printed or on your phone.

• Your order may include blood, urine, or stool tests.
• Read the preparation instructions for each test.
• Some blood tests require fasting. Follow the fasting time listed in your instructions.
• For urine or stool tests, contact your Quest Diagnostics or Sonora Quest location before collecting your sample. Ask how to get the required collection container or kit.
• Follow the laboratory’s instructions for collecting, labeling, storing, and returning your sample.
• Your tests and sample collection are already covered - you don’t need to pay again at your appointment.

Testing in Arizona? Follow the Sonora Quest instructions below.

• Wait until your lab requisition form is ready.
• Sign in to your Fullscript account.
• Open My health → Labs.
• Find your test and select View details → Schedule appointment.
• Choose a Quest Diagnostics location near you.
• Select a date and time that works best.
• Select Schedule your appointment to confirm.
• Save your confirmation email with the QR code for check-in.

For Patients in Arizona

Book your appointment directly on Sonora Quest’s website.

• Wait until your lab requisition form is ready.
• Visit Sonora Quest’s appointment page.
• For a routine blood draw, select Routine Labs as your reason for visiting.
• Choose Sonora Quest Location as your service location.
• Enter your ZIP code and select View locations.
• Choose a location, date, and time that works best.
• Sign in to your Sonora Quest account or create one.
• Select Schedule Appointment to confirm your booking.
• For urine or stool tests, confirm collection and drop-off instructions with your Sonora Quest location before your visit.

• A valid photo ID. Your name must match the information on your lab order.
• Your lab requisition form, printed or saved on your phone.
• Any samples you were instructed to collect at home, in the required containers and labeled as directed.

• Check in and present your ID and lab requisition form.
• For blood tests, a trained phlebotomist will collect your sample.
• For urine or stool tests, follow the laboratory’s instructions for collecting or handing in your sample.
• Your samples will be processed and sent to the lab for testing.

• Processing times vary depending on the tests you ordered.
• Your results will appear in your Fullscript account as they become available.
• Open My health → Labs and select View details next to your test.
• Select View results to download your report.
