This measures the percentage of your total PSA that circulates unbound to blood proteins ('free'), used together with your total PSA result to help refine prostate cancer risk assessment, particularly when total PSA falls in a borderline or 'gray zone' range. In general, a lower percentage of free PSA is associated with a higher likelihood that an elevated total PSA is related to prostate cancer, while a higher percentage is more often seen with benign prostate enlargement, though this isn't a definitive distinction on its own and is used alongside clinical evaluation and, often, a urologist's assessment.
